Monthly Mystery Tee Giveaway

We finally have the entry form back online.

(Note: we will confirm non-usa addresses via the email address used to register before processing the free Mystery Tee)

About the Monthly Mystery Tee Giveaway:
One entry per person, an once submitted, the entry is registered for future Mystery Tee drawings.

Rules and Regulations for Entering:
*Only entries submitted through this form are entered into the giveaway*

Your Entry Will Be Disqualified if:
An email is entered more that once.
A name is entered with different emails.
Multiple addresses are used under a name or email address.
Basically, any form of spamming will disqualify an entry; and possibly ban account and/or access to site.
 
How is a winner chosen?
A random number generator will be used to select the winner of the giveaway from registered entrants.
The drawing is done on the first of the month to choose the winner of previous month. i.e. (January's winner will be chosen on February 1st, March's winner will be chosen on April 1st and so on)
